The kidney plays an important
role in fluid homeostasis in the body. In patients with chronic kidney disease
(CKD) the kidney is damaged and can no longer optimally remain fluid
homeostasis in the body, leading to fluid buildup in the body, which causes heart
failure due to an increased blood pressure and problems with respiration due to
fluid retention in the lungs. Currently, the majority of patients with end
stage renal disease (ESRD) must visit the hospital multiple times during the
week (on average 3 times/week) for dialysis treatment. During this treatment a machine is
connected to the blood system of the patient to remove waste, extra chemicals
and fluid from the blood. In between two consecutive dialysis treatments, fluid
builds up in the body and during the treatment a rapid fluid shift can lead to
intradialytic hypotension. Both fluid overload and intradialytic hypotension
are associated with reduced survival rates in ESRD patients.
To date, the amount of fluid that is extracted during dialysis is based on the patients weight pre- and post-dialysis and clinically determined dry weight set points. It has been shown that bioimpedance (BioZ) is a technique that can be used to monitor fluid changes in the body and within imec wearable sensor technology has been developed that will enable continuous measurements of BioZ signals, both during the dialysis treatment as well as in a home setting in between two dialysis treatment. Monitoring of fluid status will offer the opportunity to personalize dialysis treatment to improve the quality of life for the patients and to reduce side effects of the treatment.
Data has been collected in both healthy and in patients with CKD. The goal of this project is to develop reliable algorithms and/or models for body water content estimation based on local bioimpedance measurements. Additionally, this project aims to give insight in the disease status (possibly also combining bioimpedance with additional sensor information).
This project builds on expertise of the Connected Health Solutions team. Support in this project comes from a multi-disciplinary team with expertise from hardware design to algorithm development and clinical applications.
